• Request for Hearing by Administrative Law Judge (HA-501)
  Use this form if your decision letter from the SSA requires a hearing before an Administrative Law Judge.
• Request for Reconsideration (SSA-561)
  Often filed in addition to the SSA-3441 and SSA-827 forms linked on this page, this form allows you to request the SSA to reconsider their denial of your claim.
